INSTRUCTOR PAYMENT REQUEST FORM

All contract instructors are required to use this form to submit their invoicing via email. The ACT does not accept any other form for independent contractor billing. 

Please ensure your information is entered completely and correctly  – any errors may result in a delay in processing your payment.

Classes must be completed before billing is submitted.

Email the completed form to learning@mract.org.

Request forms received before or by the deadlines will be processed on the schedule to the right.

Please be aware of submission deadlines: Submissions received after the deadlines will be paid in the following cycle.
